Diabetes - The syndrome is caused by heposecretion of antidiuretic hormone resulting from hypothalamus-pituitary lesion. Diabetes can also be secondary to other diseases. The state of illness may be mild or severe, transient or permanent. The disease belongs to the category of 'xiao ke' in Acupuncture and The Tole's Way of Diabetes Cure .

1.Diabetes - Polyuria, polydipsia and increased water intake are the principle features. If water intake is restricted, severe dehydration may occur.

2. The etiology of primary diabetes insipidus remains indeterminate. Secondary cases may be initiated by tumor, infection or trauma of the hypothalamus-pituitary system or the adjacent tissues.

3. Diabetes -Accessory examination

a) The specific gravity of urine is reduced and usually less than 1.006. The osmotic prnsipidus from psychogenic polydipsia and polyuria.

b) The osmotic pressure of plasma is elevated. There may appear dizziness, dysphoria, tachycardia or disorder of consciousness, the so-called hyperosmotic syndrome.

c) Water-deprivation test and hypertonic saline test are used to distinguish diabetes insipidus from psychogenic polydipsia and polyuria. Water- deprivation test is dangerous, and now is rarely performed.

TYPES OF DIABETES SYNDROMES and DIANOGSIS

Deficiency of the Kidney-qi Diabetes Cure . Main Symptoms : Polydipsia, frequent and profuse urination, emaciation, aching pain in the lumbus, lassitude. Diabetes Mellitus is a common metabolic endocrinopathy resulting from absolute or relative deficiency of insuline and leading to metabolic disturbance of carbohydrate, fat and protein. The Diabetes disease is frequently followed by water-electrolyte imbalance and acid-base disturbance. According to the age of the patients, clinical manifestations and requirements for insulin, diabetes can be divided into many types. 1. The characteristics of a typical case of diabetes mellitus are often polyphagia, polydipsia, polyuria and loss of body weight. Early or asymptomatic patients only show abnormal release of cortical hormone and insuline inside the body. The level of fasting blood sugar is elevated with abnormal glucose tolerance test. Symptomatic patients are frequently complicated by other symptoms of dermal, neural and endocrinous disorders, besides polyphogia, polyuria and loss of body weight, which is all under THE TOLE'S Diabetes Cure .

2. The main complications and concomitant diseases of diabetes mellitus are diabetic ketoacidosis, cardiovascular diseases, diabetic renopathy and peripheral neuropathy. Cardiovascular complications are the chief causes of death.

3. Diabetes mellitus is classified into juvenile and adult types according to the clinical features. The age of onset of the juvenile type is young and has a tendency to inheritance. Blood sugar fluctuates widely and is quite sensitive to insulin. Treatment is difficult and it is often named insulin-depending diabetes or unstable diabetes. The age of onset of adult type is above 40. This type is relatively mild and can be controlled by dietary restriction or oral antidiabetics. Therefore it is also named non- insulin depending diabetes or stable diabetes.

a) Fasting blood-glucose is higher than 130 mg. Blood glucose after meal is more than 160-180 mg. Urine is positive for glucose. If complicated by ketosis, urine is positive for ketone bodies.

b) Glucose tolerance test can be used to diagnose early or suspected cases and is the principle test in diagnosis.

c) New diagnostic techniques such as testing blood insuline levels are quite helpful in understanding the pathological changes of pancreas and in obtaining information concerning treatment.

Hypoglycaemia or low blood glucose is a condition where the level of glucose (sugar) in the blood drops below a certain point (about 3.0mmol/l). This causes a number of symptoms that usually go away 10 to 15 minutes after eating sugar.

Regular blood sugar tests can help you identify when you need to pay extra attention to your blood sugar level.

Causes

Insulin is normally produced in the pancreas and helps the body's cells absorb glucose from the blood. Normally, the glucose level rises after a meal to about 7 to 10mmol/l and insulin is secreted from the pancreas. The glucose level starts dropping again about one to two hours after the meal, and is back to normal by the next meal (about 4 to 5.5 mmol/l). By this time, the insulin level in the blood has also returned to normal.

Diabetic patients, hypoglycaemic episodes (also known as 'hypos') can be caused by too much insulin in the blood, or less insulin is needed.

Too much insulin in the blood

The dose of insulin or antidiabetic tablets is set too high.

The patient has accidentally overdosed or been given too many tablets.

Less insulin needed
1.If the patient has eaten less than usual.
2.If there has been physical activity.
If the patient has been drinking alcohol.

Hypoglycaemia can cause some or all of the following symptoms:

paleness
shaking
perspiration
a feeling of weakness
rapid heartbeat
hunger
agitation
difficulty concentrating
irritability
fatigue
blurred vision
temporary loss of consciousness
confusion
convulsions
coma.

Hypoglycaemia does not cause any of the above symptoms in some diabetic patients. This is especially the case where long-standing diabetes is concerned. In these patients, convulsions and loss of consciousness can occur without warning. To avoid this, they are asked to maintain a higher level of glucose in the blood and to measure it more frequently than is normally required. This is very important.

Hypoglycaemic episodes can be categorised as:

1. Mild hypoglycaemia or hypoglycaemic episodes. The patient can manage these episodes alone. 2. Serious hypoglycaemia. The patient will need help from others, either a family member or a doctor.

Blood glucose level is the amount of glucose (sugar) in the blood. It is also known as serum glucose level. The amount of glucose in the blood is expressed as millimoles per litre (mmol/l).

Scenario On Diabetes

On April 11, 2003 we received a visitor who introduced himself as Mr. Sureah . He came to our Tole Medical Center after reading our story on acupuncture and herbs in the local newspapers. His close friend a famous Guru and a former patient of Master Leong Hong Tole strongly recommended that he seek treatment from Master Leong and his team at his Tole Medical Center. Mr Sureah was a troubled man and from the looks on his face it was as if the world crashed on him.

Mr Sureah is an internationally well known figure in the world of Architecture and was diagnosed as having diabetes more than 2 years ago. He had a family history of diabetes whereby his father and grand father also suffered from diabetes. As such Mr Sureah must have suffered from diabetes for quite awhile and found out about it only after he went for a medical check up for some other ailment. What frightened him most was that he was not getting better despite receiving oral hypoglycemic medications. There was even a suggestion that he should go on insulin injections. Somebody told him the gory details of amputation, blindness, impotence and swelling at the lower extremities. He was already having such symptoms.

Mr Sureah is a young man in his early forties, married and have two kids. As he entered the Masters Leong Hong Tole consultation room, Master automatically knew that this person, Mr Sureah, was suffering from a chronic disease and from the looks on his face Master knew that he was also suffering from a kidney and liver problem. His body gait revealed that he was in pain.

As soon as he sat, Mr Sureah poured out all his problems and his fears. Master coolly told him that his condition was not as bad as it seemed for he had seen worse conditions before, not only from patients attending his clinic but also during his house calls. He had treated hundreds of diabetic patients having chronic complications.

Mr Sureah complained that he felt very tired and sleepy in the mornings and this was disrupting his work especially when he had to meet his clients in the morning and during discussions. His was afraid that his constant yawning might be construed as being indifferent and not interested in the discussion at hand. He joked that for most of the time his home TV would be watching him (sleeping) instead of him watching the TV. He had to wake up frequently at night, to urinate (about 3 to 5 times) and this disrupted his sleeping pattern. His cuts and bruises took a much longer time to heal. He confided that he had low sexual performance and no sexual urge for most of the time. This had created tension between him and his wife. Furthermore he easily became angry and sensitive to criticisms and at the slightest provocation. His vision was failing badly especially in the early evenings and early mornings. He complained of extreme hunger and thirst and wanted to urinate all the time. He gained weight and always lethargic in his movements. He was worried that his nervous system was failing him and that he was going to be paralyzed soon. The tingling sensation on his hands and feet confirmed such fear.

Master Leong wanted to know the type of medication that he was receiving. He told Master Leong that he received two types of hypoglycemic agents and the dosage continued to increase but the diabetic symptoms also increased. Master Leong examined his ears and eyes and asked whether he was allergic to anything. He often felt dizzy. This could be due to high sugar level in the blood, anemia, weak heart or kidney problem.

Diabetic Diagnosis

So Master Leong diagnosed Mr Sureah health conditions by checking the rhythm of his pulses. This could be done by placing his three fingers on each of Mr Sureah’s wrist. He concluded that Mr Sureah was suffering from chronic case of diabetes that not only affected the pancreas but also his kidney,Spleen, liver and heart. His body Qi was weak and blood circulation was poor. So there was also healthiness in his body. So Master had to explain to him the effect of diabetes towards the organs of his body and this affect his emotions. He also mentioned that prolonged stressful situations worsened the conditions of diabetes and would delay rate of recovery.

Master assured Mr Sureah that he will ensure that he will recover from diabetes and will soon lead a normal life. But Mr Sureah must follow his instructions to the letter especially the rescheduling of his eating habits and modification of his life styles and activities. Since Mr Sureah was never exposed to the treatment of acupuncture and herbal medicine, Master Leong had to brief him on how acupuncture and herbs worked including the role of his body Qi and blood circulation. Master had to counsel his fear of the needles.

Diabetic Treatment

Master Leong Hong Tole and his team treated Mr Sureah using the Tole Diabetic Technique and Diabetic Special Herbal Formula that had been with his family for four generations. Mr Sureah felt energized after the acupuncture treatment and he was asked to continue keeping track of his glucose levels in his blood everyday.
